If you have lost your certification card or course completion certificate, We can issue you a new one.
Before making a payment, make sure you have completed the course through our Training Center with one of our Instructors.
Please call the office to confirm your certification issue date for your replacement request at (323) 250-9919 or sent a message through “Contact Us” for an emailed response.
Once you have confirmed with one of our staff, you can make a payment.
Please make sure to include in the “Contact Us” section the card or certificate replacement type and the issue date of your certificate during the checkout.

- BLS Healthcare Provider Card
- Basic First Aid Heartsaver Card
- CA Preventative Health & Safety Certificate
- Lead Poisoning Prevention Certificate
- Pediatric CPR & First Aid Combo Card
- CPR & First Aid Heartsaver Combo Card
- If “Other,” please indicate the card or certificate type.
You may then pick up the reprint at the training center at your convenience.
Refund Policy:
No refunds will be issued on fees paid for replacement & reprints of cards and certificates.
